- a Word version to download and type into which allows you to keep an electronic copy.
Note:
- Please send two copies to Environmental and Consumer Protection Services and clearly mark the envelope with LA2003.
- You must also send a copy to Dorset Police
. - If the premises are situated in one or more licensing authority or police areas you will need to send further copies as specified on the form.
