Home from Hospital
This service provides support for a person who is going home after spending a period of time in hospital. The Home from Hospital service can give quite intensive home care support to someone whilst they make the transition back into managing in their own home and make a full recovery. Carers can find that this service helps them adjust to new demands and organise their care at home.
This service is subject to assessment by Social Services. If suitable services are identified and available they will be provided free of charge for the first 2 weeks after a person with care needs arrives home from hospital.
If, following review, a longer-term service is required a care package can be arranged to meet ongoing needs. There may be a charge for this subject to financial assessment.
To access the Home from Hospital service or to find out more information, please contact the Social Services Help Desk. ![]()
Mobile Night Care
A service which provides personal care at night. This service can also be available in an emergency. The service responds to problems as they arise and are notified to the out-of-hours team.
Free for up to 2 weeks.
If mobile night care continues after two weeks, it is charged at £50 per hour (pro rata for shorter visits, in 15-minute blocks). Many people will however be paying less than full cost because they have been assessed to pay a Fairer Charge: in this case, the mobile night care will be provided without extra charge.
Rapid Response Service
Rapid Response Service offers personal support at a time when people may otherwise not be able to care for themselves. For example, if you are ill or your informal carer becomes unwell or is absent or has gone away for a short period of time.
